OCD is a specific clinical entity, distinct from "being organized" or "perfectionistic." It responds well to treatment — usually a combination of SSRIs at therapeutic doses and a structured exposure-based therapy called ERP.

How we treat OCD here

Our medication approach for OCD involves SSRIs — often at higher doses than are used for depression — and, when indicated, augmentation with low-dose atypicals. We titrate carefully and give each trial an adequate duration.

ERP — usually by referral

The gold-standard therapy for OCD is Exposure and Response Prevention (ERP). It's a specific technique that requires a therapist trained in it. We don't do ERP in-house, but we have a short list of ERP-trained therapists in the Bay Area we refer to — and we coordinate medication care alongside their work.

Who we can't help

Severe OCD requiring intensive outpatient (IOP) or residential programs, or OCD with active suicidality, is beyond the scope of our practice. We refer to higher-level-of-care programs in the Bay Area when that's what's needed.
