THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our responsibilities
We are required by law to maintain the privacy and security of your protected health information (PHI), to notify you promptly if a breach occurs that may have compromised the privacy or security of your information, and to follow the duties and privacy practices described in this notice.
How we may use and disclose your PHI
- For treatment — within our practice and with other providers involved in your care, with your consent.
- For payment — to bill your insurance or collect payment.
- For healthcare operations — quality improvement, care coordination, training.
- As required by law — including mandated reporting, court orders, and public health reporting.
- With your written authorization — for any use not listed above, including marketing or most disclosures of psychotherapy notes.
Your rights
- Access and copies of your medical record
- Requests for corrections to your record
- An accounting of certain disclosures
- Confidential communications (by phone, email, etc.)
- Restrictions on certain uses and disclosures
- A paper copy of this notice
- To file a complaint with us or the U.S. Department of Health & Human Services without retaliation
